- Employer Identification Number (EIN)
- DUNS (data universal numbering systems) number
- List of board members.
- Financial contributions made by the board members towards the nonprofit organization.
- The overall hierarchical structure of the organization.
- Job descriptions of each employee involved in the program.
- Total number of staff members and contractors.
- Resumes of everyone involved in the program.
- Brief biographies of all the key staff members.
- Budget reports for the overall organization as well as individual programs.
- Audit Reports
- All the current sources of funding.
- Floor plan to judge how well your premises are equipped to deal with the program.
- Demonstrate compliance with various personnel-related policies.
- Disaster management plan.
- An about us description of your organization.
- All other nonprofits, government agencies, and individuals that you’re collaborating with.
- Conduct an evaluation of the program and attach the reports.
